Dry socket:
Strong pain after tooth extraction “Dry socket”
When the tooth is extracted, the jelly-like blood called blood clot blocks the affected area and protects the wound from bacteria and irritation. However, if the blood clot is not formed enough or peeled off by gargling, you may feel a strong pain. This state is called “dry socket”.
The dry socket lasts for about 2 weeks with strong pain. If so, after tooth extraction, let’s refrain from gargling, brushing, and contact with the affected area as much as possible, and try to prevent dry sockets. If you are worried, we are doing a procedure to fill the wound with collagen in advance, so please contact us.

抜歯後の強い痛み「ドライソケット」
抜歯をすると、血餅(ketsupei: blood clot)というゼリー状の血が、患部(kanbu: wound, the affected area) を塞ぎ(fusagi: block, fill up)、細菌(saikin: germ/bacteria)や刺激(shigeki: irritation, stimulation)などから傷口を守ってくれます。ところが、血餅が十分に形成(keisei: fomed)されなかったり、うがい(ugai: gargling)などで剥がれ(hagare: peeling, separated)たりすることで、強い痛いを感じる事があります。この状態を「ドライソケット」と言います。
ドライソケットは、強い痛みが約2週間ほど続きます。そうならにように抜歯後は、うがいやブラッシング・患部への接触(sesshoku: touch, contact)は極力(kyokuryoku: as much as possible)控え(hikae: refrain from)、ドライソケット防止に努めましょう(tsutomeru: try hard, work hard, make effort)。不安な方は、事前にコラーゲンを傷口に充填(juuten: fill up)する処置(shochi: measure, step, procedure)を行っていますので、ご相談下さい。

Basshi-go no tsuyoi itami `doraisoketto’
Basshi o suru to, ketsupei to iu zerii-jou no chi ga, kanbu o fusagi, saikin ya shigeki nado kara kizuguchi o mamotte kuremasu. Tokoroga, ketsupei ga juubun ni keisei sarenakattari, ugai nado de hagaretari suru koto de, tsuyoi itai o kanjiru koto ga arimasu. Kono joutai o `doraisoketto’ to iimasu. Doraisoketto wa, tsuyoi itami ga yaku 2-shuukan hodo tsudzukimasu. Sounanara ni youni basshi-go wa, ugai ya burasshingu kanbu e no sesshoku wa kyokuryoku hikae, doraisoketto boushi ni tsutomemashou. Fuanna kata wa, jizen ni koraagen o kizuguchi ni juuten suru shochi o okonatte imasunode, go sōdan kudasai.

口腔(koukuu: oral cavity)と上顎洞(jougaku hora? jougaku-dou?: upper jaw cavity / maxillary sinus)との交通
歯を抜いたあとの穴
これは上顎の親知らずの抜歯の際のリスクです。上顎の親知らずの根(ne: root)が、生まれながらにして上顎洞という空洞(kuudou: cavity, hollow)に突出(tosshutsu: portruding)している人の場合、抜歯後に抜歯窩(bassi-ka?: dental cavity)(歯を抜いたあとの穴)と上顎洞が交通し、上顎洞を間にして(ma ni shite: in between)口腔と鼻腔(bikou: nasal cavity)がつながってしまいます。そうしますと、口から飲んだ水が鼻から出たり空気が口から鼻に漏れたりします。穴の大きさによりますが、大部分(daibubun: mostly)自然に閉鎖します(heisa: close)。

This is a risk when the wisdom tooth of the upper jaw is extracted. In the case of a person whose root of the wisdom tooth of the upper jaw is born protruding into a cavity called the maxillary sinus, the dental cavity (the hole after the tooth is extracted) and the maxillary sinus will be traffic after the extraction, and the oral cavity and nasal cavity are connected between the maxillary sinus. If you do so, the water you drink from your mouth will come out of your nose and the air will leak from your mouth to your nose. It depends on the size of the hole, but it mostly closes naturally.

Kore wa jougaku no oyashirazu no basshi no sai no risukudesu. Jougaku no oyashirazu no ne ga, umarenagara ni shite jougaku hora to iu kuudou ni tosshutsu shite iru hito no baai, basshi-go ni basshi-ka(?) (Ha o nuita ato no ana) to jougaku hora ga koutsuu-shi, jougaku-hora o ma ni shite koukuu to bikou ga tsunagatte shimaimasu. Sou shimasu to, kuchi kara nonda mizu ga hana kara detari kuuki ga kuchi kara hana ni moretari shimasu. Ana no ōki-sa ni yorimasuga, daibubun shizen ni heisashimasu.

親知らずを抜いたあとはどのような症状がでますか?
symptoms after removing the wisdom teeth

  1. 腫れ hare : swelling + pain (itami)
    抜歯後12時間は腫れが急速に(kyuusoku ni: rapidly)強まり、24~36時間でほぼ(hobo: around, roughly, almost)ピークに達します(tasshimasu: reach, arrive)。その後は徐々に(jojoni: gradually)引いていき、1週間から10日かけて消失(shoushitsu: disappear, wear off)します。
    The swelling rapidly strengthens for 12 hours after tooth extraction, and it almost peaks in 24 to 36 hours. After that, it gradually retracts and disappears over a week to 10 days.
    Basshi-go juuni-jikan wa hare ga kyuusoku ni tsuyomari, 24~36-jikan de hobo piiku ni tasshimasu. Sonogo wa jojoni hiiteiki, ishuukan kara touka kakete shoushitsu shimasu.
  2. 鼻から空気や水が漏れる Hana kara kuuki ya mizu ga moreru: Air and water leak from the nose
    合併症(Gappeishou: complication)が出現し(shutsugen-shi: appear)、抜歯窩(basshi-ka? basshi-hora?: (歯を抜いた穴)が上顎洞と交通した場合には鼻から空気や水が漏れるという症状が出現します。
    Complications appear, and if the follatoma tooth extraction socket (the hole with the tooth extracted) travels communicates with the maxillary sinus, air and water leaks from the nose will appear.
    Gappeishou ga shutsugen shi, basshi-ka(?) (Ha o nuita ana) ga jougaku hora to koutsuu-shita baai ni wa hana kara kuuki ya mizu ga moreru to iu shoujou ga shutsugen shimasu.
  3. ドライソケット dry socket
    抜歯を行ったあと、通常(tsuujou: usually, normally)、抜歯窩(歯を抜いた穴)は血餅(keppei: blod clot)(血液がゼリー状に固まった(katamaru: to harden, solidify)もの)で満たされます(mitasu: to satisfy, to meet, to fulfil, to fill)。しかし、これが十分に形成されなかったり脱落(datsuraku: fall off)したりしますと、歯槽骨(shisoukotus: alveolar bone)が露出し(roshutsu: expose)、強い痛みを伴います(tomonau: to accompany)。2~5%の確率で発生するといわれ、治癒する(chiyu: healing, recovery)まで10日から2週間程度かかります。
    After a tooth is extracted, the socket (the hole where the tooth was removed) is usually filled with a blood clot (a gel-like substance made from blood). However, if this does not form sufficiently or falls off, the alveolar bone becomes exposed, causing severe pain. It is said to occur with a probability of 2-5%, and takes about 10 days to 2 weeks to heal.
    Basshi o okonatta ato, tsuujou, basshi-ka/hora? (Ha o nuita ana) wa keppei (ketsueki ga zeriijou ni katamatta mono) de mitasa remasu. Shikashi, kore ga juubun ni keisei sarenakattari datsuraku shitari shimasu to, shisou-kotsu ga roshutsu-shi, tsuyoi itami o tomonaimasu. 2~5-paasento no kakuritsu de hassei suru to iware, chiyu suru made touka kara nishuukan teido kakarimasu.

親知らずを抜いたあとの注意事項

  1. 止血 (shiketsu: stop of bleeding)
    ガーゼを咬む(kamu: bite)のは止血のためですので(圧迫止血(appaku: pressure))、20分以上しっかり咬んでください。その後は自分で取って捨ててください。その後も出血(shukketsu: bleeding)が続くようでしたら、再度新しいガーゼを30分から1時間咬んで様子をみてください。抜歯の翌日(yokujitsu: the day after)くらいまでは少量(shouryou: a small amount)の血液(ketsueki: blood)が唾液(daeki: saliva)に混じって出ますが、異常ではありません。血が出るのを気にして何回も唾液をはいたりうがいをしたりしますとよけいに(yokei ni: excessive, too much)出血しますので注意してください。ガーゼを抜歯部に当ててしっかり咬んでも、血液がガーゼに吸収し(kyuushuu: absorb)きれずにすぐに脇(waki: side, armpit)からあふれ出てくる場合は異常出血ですのですぐにご連絡ください。
    The gauze is bitten to stop the bleeding (compressive hemostasis), so bite firmly for at least 20 minutes. After that, remove it yourself and discard it. If the bleeding continues after that, bite again on a new piece of gauze for 30 to 60 minutes and see how it goes. A small amount of blood will come out mixed with your saliva until the day after the tooth extraction, but this is not abnormal. If you worry about bleeding and spit or gargle repeatedly, this will cause the bleeding to worsen, so be careful. If the gauze blood cannot be absorbed by the gauze even when you bite firmly on the extraction site, and the blood immediately overflows from your armpit, this is abnormal bleeding, so please contact us immediately.
    Gaaze wo kamu no wa shiketsu no tamedesunode (appaku shiketsu), nijuppun ijou shikkari kande kudasai. Sonogo wa jibun de totte sutete kudasai. Sonogo mo shukketsu ga tsudzuku youdeshitara, saido atarashii gaaze wo sanjuppun kara ichi-jikan kande yousu wo mite kudasai. Basshi no yokujitsu kurai made wa shouryou no ketsueki ga daeki ni majitte demasuga, ijou dewa arimasen. Chi ga deru no wo kini shite nankai mo daeki wo haitari ugai wo shitari shimasuto yokei ni shukketsu shimasunode chuui shite kudasai. Gaaze wo basshibu ni atete shikkari kandemo, ketsueki ga gaaze ni kyuushuu-shi kirezu ni sugu ni waki kara afure detekuru baai wa ijou shukketsu desunode sugu ni go renraku kudasai.
  2. 腫れや痛み
    親知らずの抜歯後は2~3日かなり顔が腫れます。腫れのピークは抜歯直後ではなく、翌日または翌々日です。とくに手術翌日の朝に突然大きく腫れていることに気がつきますが、睡眠中(suimin-chuu: while sleeping)に徐々に(jojoni: gradually)増大(zoudai: increase)したものですので心配する必要はありません。腫れは血行(kekkou: blood circulation)を良くして炎症反応(enshou hannou: inflammation response)を起こし治癒させる(chiyu: healing, cure)ために必要なものですから、冷やす必要はありません(昔言われていたことと逆ですが)。冷やしますと血行が悪くなり、治癒が遅れます。もし、腫れが気になるようでしたらマスクをかけて対応してください。疼痛(toutsuu: pan)は手術後、30分~1時間程して麻酔(masui: anesthesia)が切れたあとに生じますが、鎮痛剤(chintsuuzai: painkiller, analgesic)で十分にコントロール可能です。
    Swelling and pain
    After wisdom tooth extraction, your face will be quite swollen for 2-3 days. The swelling will not peak immediately after the extraction, but the next day or the day after. You may notice a sudden large amount of swelling the morning after the surgery, but this is due to a gradual increase while you were sleeping, so there is no need to worry. Swelling is necessary to improve blood circulation and cause an inflammatory reaction for healing, so there is no need to cool the area (contrary to what was said in the past). Cooling the area will reduce blood circulation and delay healing. If you are concerned about the swelling, wear a mask. Pain will occur after the anesthesia wears off about 30 minutes to an hour after surgery, but it can be adequately controlled with painkillers.
    Hare ya itami
    Oyashirazu no basshi-go wa 2 ~ 3-nichi kanari kao ga haremasu. Hare no piiku wa basshi chokugo dewanaku, yokujitsu matawa yoku yoku jitsudesu. Tokuni shujutsu yokujitsu no asa ni totsuzen ookiku harete iru koto ni kigatsukimasuga, suimin-chuu ni jojoni zoudai shita monodesunode shinpai suru hitsuyou wa arimasen. Hare wa kekkou wo yoku shite enshou hannou wo okoshi chiyu saseru tame ni hitsuyouna monodesukara, hiyasu hitsuyou wa arimasen (mukashi iwarete ita koto to gyakudesuga). Hiyashimasuto kekkou ga waruku nari, chiyu ga okuremasu. Moshi, hare ga kini naru youdeshitara masuku o kakete taiou shite kudasai. Toutsuu wa shujutsu-go, sanjuppun ~ ichi jikan hodo shite masui ga kireta ato ni shoujimasuga, chintsuuzai de jūbun ni kontorōru kanōdesu.
  3. 食事
    食事は手術後3~4時間してから行ってください。口の中の手術を行っていますので、普通の食事は無理です。最初は流動食(ryuudoushoku: liquid food)または半固形(han-kokei: semi-solid)食品となります。お粥やうどん、豆腐、プリン、スープ、ヨーグルト、ゼリーなどを食べるようにしてください。ミキサーを用いれば食事の種類が広がります。その後、傷の治りとともにご自分の判断で少しずつ硬いものに上げていってください。
    Meals
    You should wait 3-4 hours after surgery before eating. Since surgery has been performed inside your mouth, you will not be able to eat normal meals. At first, you will be on a liquid or semi-solid diet. Try to eat porridge, udon, tofu, pudding, soup, yogurt, jelly, etc. Using a blender will increase the variety of food you can eat. After that, as your wound heals, you can gradually increase the amount of hard foods you eat at your own discretion.
    Shokuji
    Shokuji wa shujutsu-go 3 ~ 4 jikan shite kara okonatte kudasai. Kuchi no naka no shujutsu o okonatte imasunode, futsuu no shokuji wa muridesu. Saisho wa ryuudou-shoku matawa hankokei shokuhin to narimasu. Okayu ya udon, toufu, purin, suupu, yooguruto, zerii nado o taberu youni shite kudasai. Mikisaa o mochiireba shokuji no shurui ga hirogarimasu. Sonogo, kizu no naori to tomoni go jibun no handan de sukoshizutsu katai mono ni agete itte kudasai.
  4. 歯磨き
    抜歯当日は歯磨きはせず、うがいのみにしてください。うがいを強く行いますと、出血の原因になりますので軽く行ってください。翌日からは歯磨きを行って構いませんが、抜歯部は傷がありますので1週間以上避けて(sakeru: avoid)ください。
    Brushing your teeth
    On the day of the extraction, do not brush your teeth, but only gargle. Rinsing too hard can cause bleeding, so do so lightly. You can brush your teeth from the next day, but avoid doing so for at least a week, as the area where the tooth was extracted will be wounded.
    Hamigaki
    Basshi toujitsu wa hamigaki wa sezu, ugai nomi ni shite kudasai. Ugai wo tsuyoku okonaimasu to, shukketsu no gen’in ni narimasunode karuku itte kudasai. Yokujitsu kara wa hamigaki wo okonatte kamaimasen ga, basshibu wa kizu ga arimasunode ishuukan ijou sakete kudasai.
